Can't see photos in gallery any more in most apps

This is some terrible change that showed up a few months ago and it's making me insane. I've googled a dozen times and many with the same problem and no solution! 

In these pictures you'll see what happens in reddit when I try to attach a photo, but it's the same now in every app! Let's say I want to attach a screenshot, as I just did here, 

First this recents page shows with a couple random pictures in no meaningful order, so then I click manage, then I have to go over to collections and sometimes not all the time, I can find most of my device photo folders. 

Everything has access to everything, it's not a permissions thing I can find anywhere. 

What is this new terrible gallery picker app and why did this happen?!

I literally said all the apps have been granted permission

You are right, it looks that way, but it's not. I checked every app that this happens, all have full photos permission.

That does not look like a permissions issue, rather it shows only the "Recents" as shown in the example. Click on the down arrow right of "Recents", then select a folder to choose where the images needed are and not "Media picker". The first one to choose is "Gallery" which is generally having access to all images/videos.
