This is Odin mode. It is used to do fresh software installs using a PC when there are severe issues with the software on the device.

Follow the instructions at the bottom of this screen to exit Odin mode.

How could it have occured?

 

I had switched the phone off and turned it back on 20 minutes later to see this "ODIN" screen.

I should add it was never plugged into a PC.

Most likely you accidentally turned your device on using an incorrect button combo, probably involving volume keys. The device does not necessarily need to be connected to a PC to enter download mode, that's just the intended purpose once you're in that mode.
