Phone charger exploded. 2nd in a week

Ever since last update, I've been having charging issues with my A54. Slow/fast charge is intermittent. 
Last week I had it charging and heard my Samsung block making sizzling noise. I unplugged it, it was hot. Threw it out with the cord. Today, I was charging in car on way to work, and the charging block exploded. Dangerous! Need to know if this is a battery issue? Or what could be causing this.
