"Allow notifications" grayed out but in the On position. There's no blinking, flashing or any alert when phone is asleep.

Which app? Some apps are permanently set to allow notifications and are greyed out because they cannot be disabled.  If you turn notification categories on, and there is more than one category of notification, you can disable/set the parameters (pop-up, lock screen sound) all categories.  You will not be able to disable the mandatory category Settings/notifications/advanced settings/manage notification categories for each app.  If you are not getting notifications for an app for which the notifications have been enabled, it's because the app has not sent any notifications.

any app related to phone calls. I've missed calls and no notification is shown when asleep. I assume the main app is "phone" and "allow notifications" is grayed out and cannot be changed. So far no one has been able to give me a definitive answer as to how to make it active.   I did this "Settings/notifications/advanced settings/manage notification categories for each app."  still no luck with the phone app.

If notifications are greyed out, they should be greyed out in the on position, as they cannot be turned off. Can you provide a screen shot of your notification settings?  If they are greyed out in the "on" position, make sure that sound and vibration and show as pop-up is enabled.  If this still does not work and you do not want to reset your phone, you can download and use another dialer, such as Google Phone or Phone by Google (two different dialers), and use one of those instead.
