All the latest available updates are applied.
Ever since the last system update around Jan. 23rd, my watch randomly disconnects from my phone and literally no reconnection steps work short of factory resetting the watch. Initially thought This was related to clearing system cache, but it actually occurs randomly with no specific actions by me causing it.
This has happened as often as twice in one day or up to ~1 week apart.
Prior to the last software updates, I had absolutely zero issues with my phone and watch.
Literally within days after the last software update, the random disconnects started and are still recurring.
Again, absolutely no reconnection steps work short of factory resetting the watch. At which point, re-pairing and re-setting up of the watch works perfectly until the next random disconnect between 1 to several days later.

You can try these steps for when a Bluetooth device will not stay connected to the phone: https://www.samsung.com/us/support/troubleshooting/TSG01209182/
Also try resetting the Network Settings. Phone Settings>General Management>Reset>Reset Network settings (You will need to reconnect to Wi-Fi and Bluetooth devices.)
Test in Safe Mode to see if an app is causing the disconnects. https://www.samsung.com/us/support/answer/ANS10003495/
It may also be necessary to reset both the phone and watch.
