Galaxy S25The idea behind a device ecosystem (like Samsung's or Apple's) is to use a smartphone as a hub for interoperability with your other devices.
When you switch between primary and secondary phones, you are functionally swapping out the central component of the ecosystem and thus your wearable devices need to be disconnected and reconnected when you switch between phones.
While I wouldn't anticipate substantial changes any time soon, because it would likely require a fundamental reconstruction of how connected devices function, you can request improvements to Samsung health data syncing by visiting the support tab in this app and navigating to "send feedback."

Galaxy S25A wearable can only be connected to one phone. One phone can be connected to multiple wearables. You can pair your ring with one device and your watch with another. You can have multiple wearables, each paired with a different phone, but you cannot pair all your phones to the same watch/ring

Galaxy S25As far as real time syncing between both phones, you can go into the health app (on both phones), go to the settings, tap on "sync with Samsung Cloud", and it will bring up the options. You can choose to sync using wi-fi only or wi-fi or mobile data. I believe the default is wi-fi only, so if you want to sync over mobile, you have to change the setting.
