Wondering if anyone else has experienced this issue and whether it's the phone or the app. After playing media through connected speakers, stopping the media and pressing "Stop Casting", sounds continue to come through on the speaker(s) or there is no sound at all when email or notification arrives. Each time I clear the cache of all involved apps but the issue still persists. I have to restart the phone to break the connection. I think it's the Google Home app but maybe it's the phone?

If you have a Bluetooth connected speaker, you will have to disconnect the speaker from Bluetooth.  Tap the Bluetooth icon, find your device and tap it to disconnect.

I understand what you're saying but this has been an issue fairly recently. Before OS and App updates of recent, once the media stopped playing, the speaker (s) automatically disconnected. That's why I'm asking - trying to figure out whose updates changed things

It's possible things settings got changed with the update. They often do.  I think it would be impossible to issue a list of settings changes, eliminations, movements for every update, which is why users generally have to find out for themselves and then share the knowledge.  I don't think it matters whether it was Google's or Samsung's doing.

I've found that myself about things changing during updates. The question then becomes whether the change was intentional or accidental. So knowing with whom the issue lies enables the user to research it further, find out if the change was a mistake and get it fixed if it was. And I'm sorry but don't agree with you about a list of changes. I think it is the obligation of these companies to tell you what changes are being made. Part of conducting business.

You can just put in an error report with both entities if you are not sure which one is to blame.  If you don't want to do both, put in an error report with Samsung, and they will tell you which is to blame.  It is impractical to create a list of every single settings change or movement, the update information covers what's new and major, but not the minutia.  The manual gets updated for the new features.  Sometimes the activation or deactivation of a setting is incidental, not by design.
