Did you guys flash your phones from locked U to Unlocked U1 to get Beta?
If you did and used home_csc that is what would cause this duplication as you didn't do a clean flash.

If you didn't do that and signed up for beta and received all updates without being creative then this may be bug ( I am pretty sure this is not the case )..

Also you can remove the one that's not working using one of the GoodLock modules called Registar.
