Anytime I use BT headphones, the notification volume is ear splitting loud. I can have the volume for notifications set to minimum or max, and it's always the same volume (max, to be specific). This isn't a single set or type of headphones, but has been consistent with older, newer, and different brands and types. I don't have the issue with an external speaker or with my car, just headphones. I want to hear notifications, yes, but I don't want to feel my hearing die whenever I get a text.

Try this, not 100% positive it will work, but download Good Lock's Sound Assistant.  In the advanced settings, there's a setting to get alerts only through headphones, ostensibly so if you are wearing headphones, your notifications will bypass the phone speaker and go only to the headphones.  If you turn everything off, perhaps those sounds will only come through your phone speaker (assuming you can hear the phone speaker with your music or podcast playing).  You can also try lowering the volume of the Bluetooth audio.  That slider only appears when a BT device is connected.
