Galaxy S23this is really really annoying.
i turned off note assistance or whatever and it's still doing it. i wouldn't even care, but the auto formatting function makes the formatting wrong!

With "note assit" being turned off, notes shouldn't be auto formatting.
Try clearing the samsung notes app cache by going to device settings > Apps > samsung notes > storage > tap clear cache > then restart phone.
DO NOT TAP CLEAR DATA OPTION!

Galaxy S23this still didn't work for some reason. i'm not sure what's up with my phone. i cleared cache and restarted. strange.

Try wiping the "cache partition" to see if that helps. It's safe and won't remove your settings or app files. Clearing the "cache partition" can help resolve any issues caused by old or corrupted system cache data.
Follow the steps below,
Clear the cache partition
- Turn the phone off (shut down)
- Press and hold the Volume Up key, then press and hold the side key.
- When the Android system recovery menu options show release buttons.
- Press the volume down key several times to highlight wipe cache partition.
- Press the side key to select.
- Press the volume down key to highlight yes, then press the side key to select.
- When the wipe cache partition is complete, Reboot system is now highlighted.
- Press the side key to restart the device.
GOOD LUCK!
If this doesn't work, I would send feedback to samsung cause it may be a bug in the notes app itself.
