Galaxy S22Sometimes glitches in your carrier's signal directs calls straight to voicemail without ringing the phone or showing in the call log. A lot of spammers (and legitimate companies) have ways to bypass ringing the phone and put their messages directly into your voicemailbox. Carriers themselves are notorious for this, with their marketing messages.
if all your calls are going straight to voicemail, check your settings to make sure you have no call forwarding or do not disturb on. If all that is clear, it's a carrier issue and you should contact your carrier.

Galaxy S22Oh, the ever present, yet always inconsistent "persistent voicemail notification". You can reach out to your carrier, who will then blame Samsung and when you reach out to Samsung, they will blame the carrier. In reality the tech support people at your carrier have to consult with the Samsung programmers to fix it. It does eventually get fixed, but I don't know who fixed it or it just went away on its own by serendipity. Only a handful of people have complained about this phenomenon on here that I have ever seen - me and about 3 others at various times. So, the issue is not widespread enough for anyone to really bother with except perhaps as an individual patch, specific to you. You may have some luck with asking the carrier to reset your voicemailbox to see if that gets rid of the phantom.
You can put in an error report. This is best done immediately after you get the phantom vm notification, so the logs will show the error, if any.
